A homeowner type door/window supplier should be able to get you a new window unit. You probably can't buy the frame alone.

I just did that on my steel skinned and foam insulated door.  The frame was made from plastic and I suspect it was not UV resistant because it started splitting everywhere. 
 
I also used packing tape as a temporary fix.
 
I used wood brick mold to recreate the molding.  I put the thin side to the glass and the fat side to the door skin.  I cut a rabbet to fit.  I replaced both the interior and exterior frames. 
 
On the original the molding was screwed to the steel skin on the door.  For my replacement I went through the door with wood screws and screwed into the molding on the opposite side.  I used screws both on the interior and exterior moldings. 
 
I filled the screw holes with wood  putty and painted the exterior.  I have not gotten around to painting the interior yet. 
 
It looks pretty good and the construction is robust.  I expect it to last.

The molding on mine was made from plastic.  I used the brick molding on which I cut a rabbet.

My mistake was fitting the individual pieces to the opening.  I should have assembled it into a frame first and then installed it.  I used a waterproof adhesive and I screwed through two door skins and into the molding on the other side of the door.  The plastic molding was just screwed to the door skin (light gauge steel).  Once I used the wood filler on the outside I painted and all the imperfections disappeared.  I have not painted the interior yet and that is what we are looking at here.

I tried photographing the exterior which has been finished and painted, but the glossy black paint obscures all the detail.
