Guessing you were pressing.  Twenty plus years without one flying off for me.  The flex shaft support on the rest makes supporting the tool against the work, thus contributing to friction and grab, unnecessary.  OTOH, when I was starting out, I flung several PSA and melted a bit of Velcro as well.  Not sure anyone still uses PSA types any more, which is no real loss.  Don't miss them or my finicky Masterchuck one bit!
