No issues with inserted cutters. Been using them in the metal industry for literally decades.
The balance. Non issue. Those inserts weigh very, very, very close to the same amount every time.
Screw tightening. Non issue. Tight enough with a torx screw driver is tight enough. If you want to spend the money, they sell torque screw drivers. I don't recall every having a screw I tightened loosen up.
As with anything, if you have the production to justify it, it certainly can be worth it.
