I have worked with more than one experienced person who was missing body parts.

I worked with a a fighter test pilot who once told us (engineers) the best, most experienced pilots were most at risk of an accident because they flew the airplane so close to the edge of its capabilities.  That pilot had the reputation as the most knowledgeable pilot of that particular fighter.  He's dead now, because me made a mistake and had no margin to recover.

Sounds like Joe Bill Dryden.  I flew with him in an F16 once. He was an amazing person, hard to find a more experienced pilot. But one bad decision, and ... crater.  Only F-16 to ever be lost pre-delivery. I always figured he must have been heartbroken to eject.
