If you are able to get a long driver bit and a narrow head trim screw back there I don’t think anybody would ever know.
Any free advice given is worth double price paid.

Would a pin punch get you to where you need to be? I'd put some double sided tape on it and stick the nail to it, then feed through the radiator and then hammer it in. Probably not the most elegant solution but it should allow you to get that nail in there. Or use a screw with a long driver.

If it’s nailed at each end of the rad is it going to fall off? If it’s not really tight will anyone be able to see it?

Take a metal rod and stick a rare earth magnet on it. Put nail on end of rod, thread through radiator and hammer away.
